Enclomiphene is a lab-made synthetic selective estrogen receptor modulator (SERM) — the purified trans-isomer of clomiphene citrate (Clomid). Clomiphene is a 50/50 mix of enclomiphene (the active testosterone-boosting part) and zuclomiphene (the longer-lasting estrogenic part). Enclomiphene was developed as a standalone oral capsule to raise natural testosterone in men with secondary hypogonadism while preserving fertility and avoiding the estrogenic side effects of full clomiphene.
Think of enclomiphene as a “natural testosterone booster” that tricks your brain into making more of its own hormones without shutting down production.
How it works (super simple):
• Blocks estrogen receptors in the hypothalamus and pituitary (pure anti-estrogenic effect).
• This removes the negative feedback that normally shuts down testosterone production → increased GnRH → higher LH and FSH → testes produce more testosterone and sperm.
Enclomiphene generally has a milder side-effect profile than full clomiphene because it lacks the zuclomiphene isomer.
In clinical studies: Mostly mild and transient.
Common/mild:
• Headache, nausea, or hot flashes.
• Mood changes or acne (less frequent than with clomiphene).
Less common:
• Visual disturbances (very rare compared to clomiphene).
• Potential for elevated estradiol in some men (monitored with bloodwork).
• Not for people with certain hormone-sensitive conditions without supervision.
• Unknown ultra-long-term effects (most data from shorter trials).
Bottom line: Better tolerated than clomiphene for most men; side effects are usually manageable with proper dosing and monitoring.
Typical dose
• Max 12.5mg once daily (or every other day in some protocols).
• Taken in the morning; with or without food.
For Research Patients, we strongly encourage a 6.25 EOD dosage and to take with DIM or Chrysin for mild estrogenic side-effects. Any more than this could raise estrogen high enough to warrant the use of an AI – which we want to avoid.
Enclomiphene has solid Phase 2/3 data for testosterone elevation and fertility preservation (though development stalled).
