Injectable Vitamin D3 is a sterile cholecalciferol solution at 20,000 IU per ml, supplied in a 10 ml vial (200,000 IU total). It is used in clinic protocols when oral vitamin D is a poor fit — malabsorption, inconsistent daily dosing, or a need for a known intramuscular amount — and when a recent 25-hydroxyvitamin D level shows the patient is low or needs maintenance.
Cholecalciferol is converted in the liver to 25-hydroxyvitamin D, the storage form measured on labs, then to the active hormone that regulates calcium absorption, bone mineralization, and muscle function. Injectable doses act as a depot: release is slower than an IV push and lasts longer than a single oral capsule, which is why repeat dosing is spaced in weeks, not days.
Strength
• Vitamin D3 (cholecalciferol) — 20,000 IU/ml
• 1 ml = 20,000 IU
• 2.5 ml = 50,000 IU
• 5 ml = 100,000 IU
• Full 10 ml vial = 200,000 IU
• Route: intramuscular only, unless the provider specifies otherwise
Dosing
Dose from the lab, not from symptoms. Common clinic ranges:
• Maintenance or general support: 50,000 IU (2.5 ml) intramuscularly every 2 to 4 weeks, or as directed.
• Repletion schedules (50,000–100,000 IU on a fixed plan) only when deficiency is confirmed and calcium is being monitored.
• Do not use the full 10 ml (200,000 IU) as a routine single dose unless that is the written order.
Recheck 25(OH)D and serum calcium after 8 to 12 weeks. Oil-based D3 can leave a small lump — rotate sites.
Precautions
Do not use, unless a specialist has cleared it, with high blood or urine calcium, calcium kidney stones, nephrocalcinosis, vitamin D already above target, sarcoidosis or other granulomatous disease, severe kidney impairment, or in pregnancy and breastfeeding. Check the vial oil for peanut or soy. Thiazide diuretics, digoxin, calcium supplements, and any other vitamin D product raise the risk of high calcium. Toxicity signs include nausea, constipation, strong thirst, frequent urination, weakness, confusion, and irregular heartbeat. Stop and get care if those appear.
