LL-37 (also known as human cathelicidin or CAP-18 fragment) is a lab-made synthetic 37-amino-acid peptide – the only human member of the cathelicidin family of antimicrobial peptides. It is naturally produced by neutrophils, macrophages, and epithelial cells as part of your innate immune system. The synthetic version is used in research for its powerful antimicrobial, wound-healing, and immune-modulating effects.
Think of LL-37 as your body’s ‘natural antibiotic and healer.’ It fights invaders while promoting tissue repair.
How it works:
– Disrupts microbial membranes (kills bacteria, viruses, fungi directly).
– Modulates immune response (reduces excessive inflammation while boosting healing signals).
– Promotes angiogenesis (new blood vessels) and cell migration for wound closure
In short: It kills pathogens while actively repairing tissue – a rare dual-action peptide.
LL-37 shows a good short-term safety profile in studies – generally well-tolerated.
In human pilots: No serious adverse events at research doses.
Mild/rare effects:
– Injection-site irritation or redness.
– Temporary stinging with topical use.
– Unknown long-term human effects most data from short trials or animal models.
– Theoretical risk of excessive immune stimulation in autoimmune conditions.
Bottom line: One of the milder antimicrobial peptides in research – side effects are usually minimal.
Method of Administration: Subcutaneous into fatty tissue.
Reconstitution: 2ml Bac water into single 5mg vial. See Reconstitution Guide.
Dose: 250mcg daily, therefore 10 units or 0.1ml. Use peptide calculator for alternative dosage.
Course: Will last 20 doses
Cycling: 4-8 weeks on, then 2-4 weeks off (or as needed for acute issues).
Tip: Reconstitute with bacteriostatic water; store in fridge. Use insulin syringes and sterile technique.

Your body’s built-in antibiotic – the only human cathelicidin; produced on-demand when skin or mucosa is threatened.
37-amino-acid warrior – long for a peptide, giving it broad antimicrobial punch.
Wound-healing legend – dramatically speeds closure in chronic ulcers and burns in studies.
Anti-biofilm hero – can break down protective bacterial films that antibiotics often can’t penetrate.
Research superstar – studied for everything from acne and psoriasis to gut infections and even cancer.
Non-toxic to human cells – kills microbes while sparing your own healthy tissue.
Evolutionary ancient – similar peptides exist in animals from frogs to humans - one of nature’s oldest defense systems.
Human data is limited (small pilots for wounds, skin, and infections); stronger preclinical evidence.

Takeaway: Strong preclinical and mechanistic data for infection control and healing; human trials are small but promising.
